Abstract:The purpose of this study was to determine the cause of aircraft making inadvertent or unauthorized takeoffs and transgressions onto active runways during takeoff and landing operations. The study was conducted in four phases: (1) Prior studies by FAA, NASA and MITRE/METREK were reviewed; (2) Existing data bases containing information on accidents and incidents resulting from transgressions were examined and compared; (3) Investigations were carried out by the FAA Eastern, Great Lakes and Western regions of the occurrence of transgressions at selected airports within their jurisdictions; (4) A detailed analysis was carried out for 166 occurrences drawn from the Aviation Safety Reporting System (ASRS) and the National Transportation Safety Board (NTSB) data bases. A summary of the results includes some observations with respect to possible system improvements and suggestions for further work. (Author)
