Abstract:The Federal Transit Administration (FTA) conducted an intensive investigation of passenger and employee safety on the New York Metropolitan Transportation Authority (MTA) and its operating elements. The investigation stressed the system safety concept and top-down analysis. The investigation reviewed past safety assessments conducted by state and federal agencies to ensure that all previous deficiencies have been corrected. This review was followed by an in-depth, on-site examination of each operating element. The specific related findings for each operating element were summarized in the form of a series of general conditions of concern and are presented in this report. FTA will require the MTA to develop corrective action plans to address the conditions of concern and will monitor MTA's progress in effecting the corrections.
