Abstract:This report documents an analysis and interview effort conducted to identify common operational errors made using
current Instrument Approach Plates (IAP), Standard Terminal Arrival Route (STAR) charts. Standard Instrument Departure
(SID) charts, and the preferences of pilots regarding current chart format designs. In addition, pilot preferences or
comments relevant to the presentation of IAP information in advanced electronic format were solicited and noted. The
analysis included data from prior studies and a variety of accident, incident, and operational error databases.
Representatives from flight operation user groups and charting organizations were interviewed to gain insight into
operational errors, pilot preferences and the factors which influence chart design. The IAP user groups interviewed
consisted primarily of training centers and individuals representing the full spectrum of IAP users (General Aviation,
Corporate Aviation, Air Taxi Part 135 Operators, and Air Carrier Part 121 Operators). Cartographic organizations
interviewed included Jeppesen Sanderson Inc., NOAA and the FAA. For comparison, a non-US based Carrier and chart
provider (KLM) was queried by mail.
