Abstract:The Oak Ridge National Laboratory (ORNL) Packaging and Transportation Safety Program (PATS) has been charged with the responsibility of retrieving reports and information pertaining to transportation and packaging incidents from the centralized ORPS database. These selected reports are analyzed for trends, impact on packaging and transportation operations and safety concerns, and lessons learned (LL). Moreover, the selected ORs are reviewed for compliance with the DOE Order 232.1 requirement to provide appropriate corrective actions. This task is designed (a) to keep the DOE Office of Transportation, Emergency Management, and Analytical Services (EM-76) aware of what is occurring on DOE sites and what potential transportation and packaging problems may need attention and (b) to develop and distribute LL to the Operations Offices.
This annual report details (1) the methodology that PATS uses to conduct searches of the ORPS for pertinent information and the form of reporting to EM-76, (2) major shippers of hazardous materials and major reporters of occurrences, (3) review and examination of trends observed in ORs analyzed by the nature of occurrence (NOC) codes of PATS, (4) a presentation and discussion of the root-cause codes of ORPS, and (5) evaluation of ORs that were categorized on the ORPS as emergency or unusual to determine whether the actions taken to close out the occurrences are sufficient to assure remediation of the incident and prevent recurrence.
