Abstract:This study examined why crews working in yard operations run through switches in the wrong direction or run through damaged switches. Running through a switch in the wrong direction can damage the switch. Operating through a damaged switch can derail the train. The authors worked with a railroad to collect data, interview employees, and observe yard operations to identify how and why moving equipment may run through a switch in the wrong direction. A hazard analysis called systems-theoretic process analysis was also conducted. The study identifies a complex, interacting set of factors that contribute to these events and offers
recommendations for preventing these events from occurring in the future.
