Abstract:This report examines the circumstances surrounding the loss of 4 containers of arsenic trioxide, a hazardous substance, overboard from the Panamanian M/V SANTA CLARA I during heavy weather approximately 40 miles off the New Jersey coast on January 3, 1992. 17 additional containers were also lost during the accident. While the vessel incurred minor structural damage, there were no injuries to the crew.
The Commandant concurred with the Board of Inquiry's determination that the primary cause of the cargo loss was that the cargo containers were not adequately secured on deck. Contributing causes included mechanical weaknesses in the cargo securing system and various operational oversights.
