Abstract:The objective of this audit was to evaluate the Federal Highway Administration's (FHWA) oversight of the Central Artery / Third Harbor Tunnel Project's (Project) testing procedures to ensure construction was completed in accordance with applicable specifications. FHWA provided limited oversight of the Project's testing procedures. Weaknesses were found in the Project's quality of workmanship, disposition of failed materials, implementation of the Massachusetts Highway Department's (State) Materials Manual, and completion of material documents and reports. As a result, the Project incurred $1,784,000 in additional expenses for repairs and delays due to inferior workmanship, and FHWA did not have reasonable assurance that construction and materials used in the Project were in accordance with applicable specifications.
